All Special Olympics athletes are required to have an up-to-date Medical Release and Consent Form on file with the City of Tempe delegation and Special Olympics Arizona in order to participate in official Special Olympics Arizona competitions and events. This document is good for only one year from the date of the signatures.  Athletes without an up-to-date Medical Release and Consent Form will not be allowed to participate in Special Olympics competitions, however, an athlete is welcome to continue participating in the practices without being registered for the competitions.  Please notify Adaptive Recreation at the start of the program if you choose to do this.

Filled out registrations and medical & consent forms can be mailed to City of Tempe Adaptive Recreation, 715 West 5th Street, Tempe, AZ 85281, or they can be submitted in person at the Westside Multigenerational Center located at 715 W 5th Street Tempe, AZ 85281
